2010 Required State Championships Forms - Please Read The forms required for participation have also been changed. The official State stamped team roster is to be brought to registration at State Championships before your FIRST game. Also be sure to bring along those items in the email from Carl Micke to all State Championships Managers. This form must be signed by all players who expect to participate in the 2010 event. Please include a copy of each roster that shows the name of any player used under the “club pass” provision.
The team manager and coach will be expected to sign and complete the following forms; Team Information, Eligibility form, Code of Conduct and the Game Day Roster.

These forms must be presented by the team representative at the designated “team check in” area at the tournament venue 45 minutes prior to the first scheduled game.
Teams that are scheduled to participate in a play in game must bring copies of the four forms to the Play In Game, and give to the Wisconsin Youth Soccer Official and if there is not one, give to the referees. The play in game victor must then present the original form to the “team check in” area at the tournament venue. The game day roster and player passes must be presented to the match official prior to all play in games. Remember the payment to the referee must be made before the start of the play in game. In addition the manager or coach must have in their possession the player medical consent forms and Waiver of Liability at all times.
State Championships Email List We have created an email subscription list for the State Championships. All communications regarding the tournament will be sent by email only. All team managers are required to subscribe to the State Championships Email List. Others wishing to receive email updates on the State Championships tournament are invited to subscribe to the State Championships email subscription list as well.

2010 Girls State Championships Winners
The Girls U15, U16 and U17 2010 State Championship winners have been determined. U15 Cedarburg Strikers won 2-1 over Waunakee Purple Thunder U16 FC Milwaukee Blue won 4-0 over Red Star SC U17 FC Milwaukee Blue won 1-0 over Madison 56ers Red U18 Girls FC Milwaukee Blue 9-0 Brookfield BSA Red
The champions in each age group will represent the WYSA at the U.S. Youth Soccer Region II Championships, which are scheduled for June 26 to 30 in Beavercreek, Ohio.
